NHS Test & Trace sign in

To support NHS Test & Trace (which is part of the Department for Health and Social Care) in England, we have been requested to collect and keep “a limited record of staff, customers and visitors who come on to our premises” for the purpose of contact tracing.

By maintaining records of everyone here, and sharing these with NHS Test & Trace where requested, we can help to identify people who may have been exposed to the coronavirus.

NHS Test & Trace have asked us to retain this information for 21 days from the date of your visit, to enable contact tracing to be carried out by them during that period. We will only share information with NHS Test & Trace if it is specifically requested by them.

By supplying your personal data here, you consent to our holding the data and to our releasing it to NHS Test & Trace. If you do not consent to its release to NHS Test & Trace, do not complete this form. If you do not consent to releasing your data, we do not need to hold it. Data is subject to our Data Protection policy, including the Addendum for Test & Trace data.
